In the game, the player takes control of Amicia in a third-person perspective. Most of the time, players need to use stealth to avoid hostile encounters. Amicia is equipped with a slingshot that can throw rocks to break drawbridge chains or stun guards long enough for rats to ambush them, or even kill them with a headshot.

The game features a series of survival puzzles, mainly consisting of methods to scare or distract hordes of hungry rats in order to gain access to new areas, or direct them towards enemies. The main method of keeping rats away is fire, as rats rarely enter a radius of torches and braziers. Amicia can also craft special ammunition and supplies, which include sulfur stones that start the ignition of braziers, stench bombs that attract rats, or fire suppressors to extinguish torches carried by enemies.

Amicia's younger brother Hugo can be directed to specific tasks when Amicia is busy. Later in the game, the player can take control of Hugo, who cannot create items but is able to control mice and sneak into small spaces. All of these abilities are combined when the brothers team up at the end of the game.
